Comparisons are to the median across 4,094 SMS-covered carriers in this dataset (largest carriers per state). FMCSA's national average across all ~2M registered carriers differs; the cohort median is the right comparator for a peer-cohort read.

*_ac field — what this card calls Acute/Critical — means an FMCSA investigation within the previous 12 months found an acute or critical violation in that BASIC. It is not a roadside percentile and not an intervention-threshold alert; the public file carries no percentile column, so the absence of a flag here says nothing about where this carrier ranks against its peers. Out-of-service rates are computed against driver inspections (not against power units) — drivers can be OOS for hours violations, license issues, fitness conditions, or drug/alcohol findings. For BASIC percentile rankings, FMCSA's SMS site is the canonical source (SAFER does not publish percentiles). For current insurance and operating authority status, the SAFER link above is canonical.
